Benign familial pemphigus typically occurs in intertriginous locations and is characterized by stellate erosions and maceration. Note in darker skin tones the degree of hyperpigmentation and epidermal thickening associated with this chronic condition. Apart from a biopsy, take note of the patient’s nails—if present, asymptomatic longitudinal white bands of the fingernails might be a helpful diagnostic clue.
